Leave of Absence Information

Following are the steps that must be followed for a Leave of Absence. Links to all necessary forms are included. Helpful links are listed at the bottom of this page.

STEP 1:

 

A District employee who is going to be absent for more than five consecutive working days must complete a Leave of Absence application prior to the absence. The following forms must also be completed and be attached to the application:

If the employee is an active member of the Sick Leave Bank and has exhausted all of his or her Annual Leave and Vacation, he or she may include a completed Sick Leave Bank request form with the Leave of Absence application if desired. STEP 3:

 

Any employee (whether an active member of the Sick Leave Bank or not) who has exhausted all of his or her Annual Leave and Vacation may include a completed Compassionate Leave application with the Leave of Absence application if desired. NOTE: The Sick Leave Bank Committee meets the first Monday of each month to review Sick Leave Bank and Compassionate Leave requests.

STEP 4:

 

The employee is responsible for obtaining his or her Supervising Administrator's signature on the Leave of Absense application and any other necessary forms denoting the Supervising Administrator's approval of the absence.

STEP 5:

 

Once all forms are complete and signed by the Supervising Administrator, the employee must submit the entire packet to the Human Capital Officer. To do this, the employee should contact Fara Grange, HC Assistant-Employee Attendance/Substitute Services, at 579-2026 or via email to set up a meeting to review the documents and ensure that they are completed correctly. The packet will then be forwarded to the Human Capital Officer for consideration.

STEP 6:

 

The employee will be notified as soon as possible regarding the acceptance or denial of his or her Leave of Absence application.

STEP 7:

 

If approved, it is the employee's responsibility to report days missed in the SAM system for the duration of the leave.

STEP 8:

 

If the employee is unable to return to work on the date stated in the approved Leave of Absence application, it is the employee's responsibility to apply for an extension.

STEP 9:

 

Prior to returning to work, employees must contact the HC Department to confirm the return-to-work date and ensure payroll reinstatement. Points of contact are:
