  • When submitting mileage please ensure that each month is submitted on a separate form.
  • Mileage should be submitted monthly.
  • Please make sure that forms have correct account numbers and approvals. Forms with missing information will be returned.
  • Please ensure that your vendor number is included on the form. YOUR EMPLOYEE NUMBER IS NOT YOUR VENDOR NUMBER.
  • All administrators submitting mileage for reimbursement need to have their form approved by Mr. Miles.
