OPEN ENROLLMENT BEGINS MONDAY, APRIL 15, 2013 AT 7:30 A.M. AND ENDS ON FRIDAY, MAY 10, 2013 AT 4:00 P.M. PACKETS WILL START GOING OUT TO THE BUILDINGS THE WEEK OF APRIL 1.
ANYONE WHO HAS NOT MET WITH AMERICAN FIDELITY TO COMPLETE THE MANDATORY SECTION 125 PART OF OPEN ENROLLMENT NEEDS TO CALL ANGELA AT 579-2037 AND SCHEDULE AN APPOINTMENT THE WEEK OF MAY 6-10.
EMPLOYEES THAT FAIL TO MEET WITH AMERICAN FIDELITY WILL HAVE THEIR DEDUCTIONS CONVERTED TO AN "AFTER-TAX" STATUS EFFECTIVE WITH THEIR JUNE 25, 2013 PAYROLL.

If you need assistance when enrolling in benefits through the e-portal please contact the Benefits office. You cannot enroll for the Anthem Blue Cross & Blue Shield voluntary/additional products through the e-portal, this must be done via paper enrollment form obtained from the Benefits office.
